I ran across an old copy of a UPS airline gripe sheet. After every flight the pilot fill out a form called a gripe sheet, which tells the mechanics about problems with the aircraft. The mechanics correct and document the problems and the pilots can review them before the next flight.
P; Left inside main tire almost needs replacement
M; Almost replaced left inside tire.
P; Test Flight OK, except auto-land very rough.
M; Probably because auto-land is not installed on this aircraft

BTW UPS is one airline that has never had an accident. One wonders
